The Impact of Social Determinants on Black Family Health
Social determinants of health (SDOH), such as limited healthcare access, economic barriers, and systemic inequities, have placed Black families at the top of the charts for chronic conditions like hypertension, diabetes, and obesity. These disparities result in higher rates of preventable hospitalizations and increased healthcare costs for insurers, municipalities, and providers.
Chronic Disease Statistics
➺ Black adults are 60% more likely to be diagnosed with diabetes than White adults (CDC).
➺ Hypertension affects nearly 58% of Black adults, compared to 43% of White adults.

Why
It Matters
Black families disproportionately experience chronic disease due to barriers in health education and healthcare access. Michelle’s work directly addresses these challenges by providing easily accessible health education that promotes equity and supports systemic change. Her programs empower families to take control of their health, while also reducing the financial burden on healthcare systems.
For over 28 years, I’ve had the privilege of working in healthcare and witnessing firsthand the disparities that exist within our system. But what truly fuels my passion is the realization that so many communities—especially communities of color—are not receiving the health education and resources they deserve. I’ve seen how systemic inequities, cultural barriers, and a lack of accessible information have created significant health gaps, and I’ve made it my life’s work to change that.
I do this work because I believe in the power of knowledge. I believe that when people have access to the right information—when they are empowered to make informed choices about their health—they can transform their lives. My mission is simple: to bridge the wellness knowledge gap and create spaces where health education is accessible, relatable, and truly impactful.
I do this work because I understand the deep connection between culture and health. As a nurse and healthcare leader, I’ve seen that wellness isn’t a one-size-fits-all approach. What works for one community may not resonate with another. I’m committed to delivering health solutions that are culturally relevant, ensuring that every individual feels seen, heard, and supported in their wellness journey.
I do this work because I know that true health isn’t just physical—it's holistic. It’s about mental, emotional, and social well-being, too. Whether I’m speaking at an event, writing an article for The Color of Wellness Magazine, or working with organizations to create meaningful wellness programs, my goal is always the same: to inspire individuals and organizations to embrace holistic wellness, because I believe we all deserve to live healthier, happier lives.
